What’s different about VitellaCare managed by Crossover?
We believe each person’s health journey requires a personalized plan. Our primary care provider-led teams include nurses, doctors and mental health providers that work together onsite to offer custom, preventive and proactive health management. The holistic, collaborative whole-health team approach empowers you to manage your health confidently and effectively with support and guidance.
How do we simplify healthcare?
We are dedicated to making your health journey easier, with less stress. We offer same or next-day appointments, in-person or virtual and Care Navigators to help with all your questions. Our centers are equipped to offer a broad range of primary care services, including mental health services, designed to streamline and improve your experience.
What does a visit look like?
During your visit, you’ll stay in one room and care will come to you as needed. Your provider will spend time listening to your concerns. They will collaborate with other providers to develop a care plan that may include onsite blood draws or scheduling a mental health check-in. Our approach ensures a thorough, very personal healthcare experience, leaving you with a clear plan and peace of mind.
Does VitellaCare provide virtual or in-person care?
Both. For online care, you can message back and forth with your care team anytime on the patient portal or you can schedule video appointments, when needed. Virtual care options are available for members who are physically residing in Minnesota (MN) or North Dakota (ND) at the time of the virtual visit. If in-person care is needed and you do not live near a health center, your VitellaCare Care Navigator will help you schedule in-person visits within community providers and arrange for any needed tests or lab work.
How is this different from typical virtual care providers?
With most virtual care providers, you get a new doctor for every visit— making it difficult to have meaningful conversations about your ongoing medical needs. With VitellaCare, you will see the same primary care doctor, mental health therapist, or other provider each time you need care. Moreover, your VitellaCare care team will proactively reach out to you to be sure you are feeling better, answer questions about your care plan, and help you maintain a healthy lifestyle. Unlike other virtual care providers, this model supports long-term relationships that ultimately result in better health outcomes.
How does this work with my extended health network?
VitellaCare, managed by Crossover Health, ensures a smooth connection to other health networks, giving you access to full care beyond the VitellaCare center. Health records are shared easily, ensuring your care is consistent and well-managed.
What about costs?
We know the cost of healthcare is challenging, but VitellaCare is committed to simple pricing and billing. You can expect predictable fees to ensure consistent costs.
VitellaCare aims to reduce healthcare costs through its focus on preventive care and proactive health management. Similar centers have achieved up to 15% reduction in a client’s total cost of care by reducing the need for more expensive emergency and specialty care and increasing the quality of care.
Can businesses purchase health plans to access VitellaCare?
Yes. Self-funded health plans are encouraged to talk with their broker or BCBSND representative to discover potential cost savings through this primary care model for their employees. Starting January 1, 2025, these centers will be built into eligible BCBSND fully insured health plans. The VitellaCare model, through Crossover's management approach, is a highly rated employee benefit with sustained 93% member satisfaction, improved population health outcomes, and lower healthcare costs.
